Obtaining U.S. Birth Certificates and other Vital Records

Vital Records are documents such as birth, death or marriage certificates. In the U.S., these documents are generally recorded and issued by individual state (or county or city) governments, not the federal government.  Therefore, copies of such Vital Records must be obtained from the appropriate state (or county or city) in which the event occurred

For information on how to obtain Vital Records documents from various places in the U.S., visit http://www.cdc.gov/nchs/howto/w2w/w2welcom.htm

Please Note: Slovak authorities often require a U.S. vital record document be “legalized” for use in Slovakia by obtaining a certification called an “Apostille.”  For further information on how to obtain an apostille, click here
